Existence of antimicrobial resistance (AMR) has been a global issue as it brings threat to human health. Under Malaysia Action Plan on AMR (MyAP-AMR), Malaysia has been chosen as one of the pioneer in pilot project conducted by WHO named WHO Integrated Global Survey on Extended Spectrum Beta Lactamase producing E. coli (Tricycle Project) which involved multiple agencies from various ministries. Started at early 2018, the project aims to build an easy, integrated and multisector in monitoring system on AMR. These project consists of three main components; human, food chain and environment. NAHRIM, through Water Quality Laboratory was involved in this project for environment sector in which generally to monitor the existence of resistance E. coli in river, waste water and food chain waste (poultry slaughterhouse).
In conjunction to the event, an excursion by the WHO experts was performed on 12-14 September 2018 to ensure the implementation of the project based on the module. The experts consist of; Dr Awa Aidara-Kane (WHO Geneva), Dr Heike Schmitt (Dutch National Institute for Public Heatlh and Environment, Netherland) and Dr Shivaramu K. Veerappa (North Carolina State University). Opening ceremony of the event started with a talk by Dr Aidara-Kane; to introduce the project background on AMR issues worldwide followed by implementation progress presentation by the principal investigator, Dr Suraya Amir Husin (Ministry of Health). NAHRIM was represented by Deputy Director General, Ir. Hj. Mohd Fauzi Mohamed and Research Officers from Water Quality Laboratory; Puan Syazrin Syima Sharifuddin, En Muhammad Azroie Mohamed Yusoff and Dr. Noor Haza Fazlin Hashim.
On the second day, the excursion started with a visit to Bacteriology Unit, Kuala Lumpur Hospital. Presentation on project implementation progress was given by Datin Dr. Salbiah, Head of Microbiology Department followed by laboratory tour. After that, the excursion continue to visit Bacteriology Unit, Institute of Medical Research (IMR) where briefing on the project implementation was given by Dr Norazah Ahmad, Head of Infectious Disease Research Centre followed by lab excursion. Second day visit ended by excursion to National Public Health Laboratory, Sungai Buloh. The project inplementetation progress was given by Mrs Pusparani A/P Ramasamy, Science Officer of Bacteriology Unit followed by laboratory excursion.logi.
Third day program started with a visit to National Veterinary Public Health, Salak Tinggi. Briefing on the project implementation was given by Dr Noor Asyikin Abu, Veterinary Officer prior to lab excursion. Next, the excursion came to NAHRIM for introductory briefing and laboratory visit presentation on project implementation was given by Dr Noor Haza Fazlin Hashim and En. Muhammad Azroie Mohamed Yusoff, Research Officers from Microbiology Unit. The presentation touched on sampling techniques, criteria on choosing sampling sites and current progress on the project.
The program ended in the afternoon where a roundtable discussion was done basically on the findings from the visit. Overall, the experts were satisfied with the progress achieved from the program. They also praised on the efficient synchronization and cooperation among the group members consisting multiple agencies. The experts also suggest to carry out antibiotic residual analysis from the water samples and hopes this project will a continuous project in Malaysia in order to build a surveillance program on AMR in Malaysia.
